What is semaglutide?

Semaglutide is the active ingredient in Wegovy®, which is FDA approved for weight management as a weekly injection and, since December 2025, a daily pill. Ozempic® and Rybelsus® contain it too, but are approved for type 2 diabetes.

Semaglutide also comes as compounded versions from state-licensed pharmacies, typically at a lower monthly cost. The 2024 to 2025 shortages are now resolved; compounded versions remain available through state-licensed (503A) pharmacies, prepared for an individual patient with a medical need the standard product doesn't meet.

What do the studies show?

In the main trial behind Wegovy®, people lost about 15% of their body weight over 68 weeks, compared with about 2% on a placebo. In adults with heart disease and excess weight, it also lowered the risk of heart attack, stroke and death from heart disease by 20%.

These results belong to Wegovy®. Compounded semaglutide has not been tested in trials.

Is semaglutide safe for weight loss?

Wegovy® is FDA approved for long-term weight management and has been extensively studied for safety and efficacy.

The most common side effects are nausea, diarrhea, vomiting and constipation, mostly while your dose is rising, and they usually ease over time.

It carries a boxed warning about thyroid tumors seen in animal studies, and it is not for anyone with a personal or family history of medullary thyroid cancer or MEN 2, or who is pregnant.

Other warnings include pancreatitis and gallbladder problems, and a higher risk of low blood sugar when combined with insulin or a sulfonylurea.

How much weight can I lose on semaglutide?

In the main trial behind Wegovy®, people lost about 15% of their body weight on average over 68 weeks, compared with about 2% on a placebo.

On the higher 7.2 mg dose, approved in March 2026, people lost about 19% over 72 weeks, and on the Wegovy® pill about 14% over 64 weeks.

Your results depend on your dose, how long you stay on treatment and changes to diet and activity.

How much does semaglutide cost?

Without insurance, Wegovy® costs $349 a month from its maker, and new patients pay $199 for each of their first two fills through December 2026.

The Wegovy® pill costs $149 to $299 a month, depending on the dose.

Compounded semaglutide starts at about $49 to $69 a month at our top three programs, with the clinician and the medication included.

Does insurance cover semaglutide for weight loss?

Some plans cover Wegovy® for weight loss, usually after a prior authorization that checks your body mass index and related conditions.

Medicare's GLP-1 Bridge program covers it for a $50 copay through December 2027, for people who qualify.

Compounded semaglutide is almost never covered.
