What is tirzepatide?

Tirzepatide is the active ingredient in Zepbound®, which is FDA approved for weight management and for obstructive sleep apnea in adults with obesity. Mounjaro® contains it too, but is approved for type 2 diabetes. Both are weekly injections.

Tirzepatide also comes as compounded versions from state-licensed pharmacies, typically at a lower monthly cost. The 2024 to 2025 shortages are now resolved; compounded versions remain available through state-licensed (503A) pharmacies, prepared for an individual patient with a medical need the standard product doesn't meet.

In the main trial behind Zepbound®, people on the highest dose lost about 21% of their body weight over 72 weeks, compared with about 3% on a placebo. In the only trial to compare the two directly, which tirzepatide's maker funded, people lost about 20% on tirzepatide and about 14% on semaglutide.

These results belong to Zepbound®. Compounded tirzepatide has not been tested in trials.

Is tirzepatide safe for weight loss?

Zepbound® is FDA approved for long-term weight management and has been extensively studied for safety and efficacy.

The most common side effects are nausea, diarrhea, vomiting and constipation, mostly while your dose is rising, and they usually ease over time.

It carries a boxed warning about thyroid tumors seen in animal studies, and it is not for anyone with a personal or family history of medullary thyroid cancer or MEN 2, or who is pregnant.

Other warnings include pancreatitis and gallbladder problems, and a higher risk of low blood sugar when combined with insulin or a sulfonylurea.

It can make birth control pills less effective for four weeks after you start and after each dose increase, so ask your clinician about a backup method.

In the main trial behind Zepbound®, people lost about 15% of their body weight on average at the 5 mg dose and about 21% at 15 mg over 72 weeks, compared with about 3% on a placebo.

Your results depend on your dose, how long you stay on treatment and changes to diet and activity.

Is tirzepatide better than semaglutide for weight loss?

For weight loss alone, the evidence favors tirzepatide.

In the only trial to compare them directly, which tirzepatide's maker funded, people lost about 20% of their body weight on tirzepatide and about 14% on semaglutide over 72 weeks.

That trial did not include semaglutide's newer 7.2 mg dose.

Semaglutide also comes as a daily pill, and Wegovy® is approved to lower the risk of heart attack and stroke in people with heart disease, so your clinician can help you decide.

How much does tirzepatide cost?

Without insurance, Zepbound® costs $299 to $449 a month from its maker, depending on the dose, as long as you refill within 45 days.

Does insurance cover tirzepatide for weight loss?
