Starting a new medicine is easier when you know what's coming.
Here's a friendly walk-through of tirzepatide, the medicine in Zepbound® and Mounjaro®, from your first shot to the months that follow.
01The basics
What is tirzepatide?
Tirzepatide is a once-a-week shot for weight loss. It copies two hormones your gut releases after you eat, so you feel full sooner and less hungry between meals.1U.S. Food and Drug AdministrationSourceFDA prescribing information for Zepbound® and Mounjaro®, with the Zepbound® Medication Guide and Instructions for Use: Dosage and Administration; Adverse Reactions; Drug Interactions; Clinical Pharmacology; Clinical Studies; How Supplied, Storage and Handling. All sources
It's sold as Zepbound®, approved for weight loss, and as Mounjaro®, approved for type 2 diabetes. They're the same medicine at the same strengths, so you'd only ever take one.1U.S. All sources
Whichever you take, a licensed clinician reviews your health first and sets your dose.
02Your weekly shot
How do you take tirzepatide?

You take it once a week, on the same day, at any time of day, with or without food.1U.S. All sources
Zepbound® comes in a few forms. There's a single-dose pen with the needle built in, a vial you draw up with a syringe, and a KwikPen that holds four weekly doses.1U.S. All sources
Ask your program or pharmacist to show you how yours works before your first dose. Air bubbles are normal, and there's no need to warm the pen first.1U.S. All sources
You inject it just under the skin of your stomach or thigh. If someone helps you, the back of your upper arm works too.1U.S. All sources

03The dose ramp
How does the tirzepatide dose go up?
Slowly, on purpose. Starting low and stepping up gives your body time to adjust, which keeps side effects gentler.1U.S. All sources
Weeks 1 to 4: 2.5 mg

05Side effects
What side effects are common, and what helps?
The most common side effects are stomach-related, like nausea, diarrhea, constipation and vomiting. Tirzepatide is usually a long-term treatment, and the results last while you take it. 07Getting started
How do you get started with tirzepatide?
You'll need a prescription. Many people start with an online program: you answer some health questions, and a licensed clinician decides whether tirzepatide fits.
Paying cash, Zepbound® vials and KwikPens cost $299 a month for 2.5 mg and $399 for 5 mg. Higher doses are $449 when you refill on time, plus needles or syringes.11Eli Lilly and CompanySourceLillyDirect. Zepbound® (tirzepatide) self-pay prices. Checked October 6, 2026.Open the source All sources
Compounded tirzepatide at the programs we rank started at $89 a month, depending on the program and plan. We checked in late September and early October 2026.

Frequently asked questions.
Can I stay on 2.5 mg?
It's a starting dose, not one meant for the long term.
After four weeks, the plan is to move up to 5 mg, your first full dose.
If you're struggling, your clinician can give you more time before a step.1U.S. All sources
Does the shot hurt?
It's a small needle just under the skin, and the single-dose pen has it built in, so there's nothing to attach.
In studies, fewer than 1 in 10 people had a reaction where they injected, like redness or itching.1U.S. All sources
Why does my hunger come back before my next shot?
Some people notice their appetite creeping back toward the end of the week.
Stick to your weekly schedule rather than taking a dose early, and mention it to your clinician, who can adjust your plan.1U.S. All sources

If it's been longer, skip it and take your next dose on your usual day.1U.S. All sources
Does tirzepatide affect birth control pills?
It can make them less effective for a while.
Use a backup method, or switch to a non-pill method, for four weeks after you start and for four weeks after each dose increase.1U.S. All sources
How is compounded tirzepatide taken?
Usually as a weekly shot, often from a vial that holds several doses, with your dose written in units on a syringe.
Units are a measure of volume, not milligrams, so ask your clinician or pharmacist to show you exactly how much to draw.13U.S. Food and Drug AdministrationSourceFDA. FDA's Concerns with Unapproved GLP-1 Drugs Used for Weight Loss. Content current as of October 1, 2026.Open the source All sources
