If you've shopped around for a GLP-1, you've probably seen two kinds: brand names like Wegovy® and Zepbound®, and compounded versions, often for less.

So which is right for you? It depends on your coverage, your budget and what you'd like, and you don't have to work it out alone. Here's a friendly side-by-side to get you started.

01The short answer

What's the difference between branded and compounded GLP-1s?

Brand-name GLP-1s come from the companies that developed them, Novo Nordisk and Eli Lilly. The FDA approved each one after large studies.U.S. Food and Drug AdministrationSourceFDA prescribing information for Wegovy® (injection and tablets), Zepbound® and Foundayo™: Indications and Usage; Dosage and Administration; Clinical Studies; How Supplied. All sources

02Cost

How do the prices compare?

Brand-name prices have come down a lot, and the pills are now the lowest-priced brand-name option. Here's what you'd pay in cash:

At the programs we rank, the compounded price usually includes your clinician too, and several keep one price at every dose. We checked in early October 2026.

If you get brand-name medicine through an online program, there's usually a membership fee on top of the maker's price.

So the gap is smaller than it used to be, especially at starting doses. A handy tip: price out the dose you'll likely stay on, not just your first month.

You'll usually get a compounded version through an online program, which connects you with a clinician and ships from a partner pharmacy. Most of the programs we rank offer compounded semaglutide and tirzepatide, and some offer brand-name medicine too.

07Choosing

Which is right for you?

It depends on your coverage, your budget and what you'd like. Here's how it often shakes out:

  1. Your insurance covers it

Brand-name may cost you the least, especially with a savings card or Medicare's Bridge.Novo NordiskSourceNovoCare. Wegovy® savings offer terms and conditions. Checked October 6, 2026.Open the source All sourcesCenters for Medicare and Medicaid ServicesSourceMedicare.gov. Weight loss drugs: the Medicare GLP-1 Bridge. Checked October 7, 2026.Open the source All sources

  2. You're paying cash

Compounded usually costs less, and several programs keep one price at every dose. The brand-name pills start at $149 a month.Novo NordiskSourceNovoCare. Wegovy® (semaglutide) self-pay prices. Checked October 7, 2026.Open the source All sources

  3. You'd like a pen or a pill

Brand-name pens come prefilled, so there's nothing to draw up, and the brand-name pills are daily tablets.U.S. Food and Drug AdministrationSourceFDA prescribing information for Wegovy® (injection and tablets), Zepbound® and Foundayo™: Indications and Usage; Dosage and Administration; Clinical Studies; How Supplied. All sources

  4. You're still not sure

That's what your clinician is for. They'll weigh your health, budget and preferences with you.

Good questions to bring to your clinician: Which suits my budget and coverage? If it's compounded, which pharmacy makes it? How will my dose be measured?

Whichever you choose, look for a program with a real clinician, a licensed pharmacy you can look up, clear dosing help and clear pricing.

08Switching

Can you switch between them?

Yes, with a new prescription from your clinician, and lots of people do. In a 2026 survey, over a third of people on a compounded version had switched from a brand, often over cost or coverage.GallupSourceGallup. In U.S., GLP-1 usage reaches new high. July 7, 2026.Open the source All sources

The brand-name labels don't cover switching from a compounded version.
